Jacob has hydrocephalus, spina bifida and Arnold-chiari malformation. Heâ€™s tube fed and catheterised.
Heâ€™s seven years old and loves going in the car.
His favourite colour is yellow and he loves elephants and dinosaurs.
Heâ€™s a happy smiling boy
